Women’s Experiences Following Hysterectomy Psychological and Relational Impact and Perceptions of Nursing Care. A Mixed-Methods Study

Abstract
Background: Hysterectomy is a widely performed gynaecological procedure that represents a definitive treatment for various uterine conditions, both benign and malignant. However, the intervention may have a significant impact on the relational and psychological spheres of women, as it involves the loss of an organ closely associated with femininity, motherhood, and body image. Interest in this topic arises from the need to better understand how women experience this surgical procedure, particularly in relation to quality of life, feminine identity, and the role of nursing care.
Objective: To explore the lived experiences of women undergoing hysterectomy, with a focus on emotional, psychological, and relational aspects. Particular attention is given to the role of nurses in providing emotional, informational, and relational support.
Methods/Materials: A mixed-methods study was conducted between February and May 2025 in two hospitals in the province of Cagliari. The sample consisted of 50 women: 30 participated in the quantitative phase through a structured closed-ended questionnaire, and 20 in the qualitative phase through semi-structured interviews. Quantitative data were analysed using descriptive statistics and the chi-square test, while qualitative data were examined through thematic analysis.
Results: Most participants reported a positive evaluation of nursing care, particularly regarding staff availability, professional competence, and post-operative pain management. No statistically significant association was found between age and difficulty in accepting body image (p = 0.247). Qualitative analysis revealed experiences of fear, vulnerability, and symbolic loss, often accompanied by feelings of relief and a perceived improvement in quality of life following surgery.
Conclusions: Both qualitative and quantitative findings confirm that hysterectomy represents a complex experience involving physical, emotional, and relational dimensions. Nursing care emerges as a fundamental component in promoting women’s well-being and in supporting the experience as a process of care and recovery. The absence of a statistical correlation between age and adaptation difficulties highlights the need to provide psychological support to women across all age groups.
