Abstract
L'onicomicosi cronica, comune a molti pazienti, è causata principalmente da dermatofiti, lieviti e muffe non dermatofitiche. Circa il 90% dei casi di onicomicosi delle unghie dei piedi è dovuto a dermatofiti, in particolare Trichophyton (T.) rubrum e T. mentagrophytes.
Nonostante la disponibilità di numerosi farmaci antimicotici, fattori complessi come la resistenza ai farmaci, la formazione di biofilm, la scarsa penetrazione del farmaco nell'unghia e le condizioni individuali del paziente rendono difficile una guarigione completa e definitiva.
Questo articolo sottolinea l'importanza di nuovi approcci terapeutici e potenziali direzioni di ricerca, esaminando al contempo le complesse cause della resistenza delle infezioni fungine delle unghie dei piedi al trattamento a lungo termine.
